Eli didnt eat candy because she lives off blood. Her body would reject anything else. Just how a Lion wouldnt be about to eat grass. It is interesting that in the movie she tries it only to make Oskar happy. While in the book she flat out rejects it.

As for Eli bathing I think over the many years Eli didnt feel the need to. Just like not wearing socks and shoes in the snow. Or a jacket.

In the castration part, Oskar feels a red hot poker sticking through his groin and making it's way up to his chest. Is this just a metaphor for the pain or did the madmen really stabbed Elias that profoundly? Also at the end it says that man with the wig "bites and drinks" is he eating Elias's groin or is he bitting Elias?

The " red hot poker" was a metaphor for the degree of pain and suffering. I believe "bites and drinks" is a reference to blood drinking only and nothing else. :)
